New Patients
- Prior to your first appointment, patients should have all patient information forms completed, signed and bring to your appointment.
- Bring your insurance cards
- If your insurance requires a referral, please notify your primary care physician to obtain a referral and bring it with you or arrange to have your physician's office send or fax it prior to your appointment. If you cannot provide this information by your appointment date, please call our office.
- Obtain pertinent medical records, x-rays, MRI, and scans previously completed for this condition or injury
- If you are being seen under a Workers’ Compensation claim, it is important that we have your claim number, date of injury, employer at the time of injury, and all conditions. Prior to your appointment our office will need written authorization from your worker compensation company or your employer. If you cannot provide this information by your appointment date, please call our office.
- Be prepared to pay your financial responsibility portion at the time of service.
Current Patients
If you have not been seen in the past six months, you need to update your personal information by downloading and completing the appropriate forms. Please give the completed forms to the receptionists when you arrive at the office.

Payment for your deductibles and co-payments based on your insurance benefit plans is expected to be paid at the time of service. We will also work with you to calculate a pre-payment deposit for diagnostic tests and surgery procedures. We offer various methods of payment including cash, checks, debit cards and credit cards, including MasterCard, Visa, American Express.
We will gladly submit your charges to your insurance carrier(s). Please keep in mind that you, as the patient, are ultimately responsible for your bill. The billing department will be glad to answer any questions or concerns regarding your insurance and billing.
Please keep in mind that Medicare and other insurance companies may not pay for durable medical goods (ace wrap, foot supplies, wheelchair cushions, canes, etc.). It is a good idea to become familiar with the rules of your health plan prior to your treatment.

Prescription Policy
It is our office policy to refill all medications during office hours only. If you call in the evenings or on the weekend, the answering service will not forward your calls to the physician on call. We need to keep an accurate record of all medications prescribed to stay within state and federal laws and prescribing guidelines. Therefore, all requests for medications must be called into our office Monday no later than 10 AM on Friday. All requests after this time will not be processed until the next business day.
After hours prescription refills
No prescriptions will be filled outside normal office hours, except for emergencies. Lost prescriptions are rarely considered to be an emergency. No prescriptions for narcotic medications will be filled for patients whose medical conditions are not well known to the physician on call.
After Hours Care
If you need to contact our physicians about urgent matters after the office is closed, please call our office telephone number, which will be answered by our answering service, and the on-call physician will be given your message.
You may also choose to be seen in the emergency room, where records of your care will be forwarded to us.
